Life Insurance

Mutual of Omaha, Policy #G000CF5T

  • Tri-State provides basic life insurance and accidental death and dismemberment (AD&D) policy for each eligible employee at no cost to you or your dependents. The policy provides life and AD&D coverage of $20,000 for employees, $5,000 for spouses, and $2,500 for dependent children.

    • Please note that it is required for you to enter your spouse and children's information in SyncHR for them to receive this benefit. Again, this is at no cost to you.

  • In addition to the company-provided group life insurance, we are happy to offer employees the ability to purchase additional coverage to supplement the company provided through Mutual of Omaha. Employees purchasing supplemental life through Mutual of Omaha are able to secure additional Life Insurance at the lower group life rates versus purchasing on the open market. All coverage offers Portability, so you can keep your coverage, even if you were to leave Tri-State. Employees may purchase up to 5x their annual salary up to $500,000. You may purchase for a spouse 100% of your employee benefit up to $200,000. For a child you may purchase 100% of the employee benefit up to $20,000. 

    • ***Note: when it says you may purchase 100% of the employee’s benefit, this means that the amount you are insuring a spouse or child must not exceed the amount you purchase for yourself as an individual.

Disability

Short-Term Disability Insurance replaces a portion of your income when you are unable to work due to illness or injury. Tri-State covers the cost of this coverage for all employees. This coverage provides a weekly benefit that equals 60% of your weekly earnings up to a $500 weekly maximum. Benefits become available after you have been disabled for seven (7) days due to illness and injury (benefits begin on the 8th day). Benefits are payable for a maximum duration of 13 weeks.

Long-Term Disability: If you remain disabled for 90 consecutive days due to any one disabling illness or injury, you will be eligible to receive Long-Term Disability benefits. If approved by the insurance carrier, this coverage will provide a continued source of income that equals 60% of your before-tax monthly earnings to a maximum of $5,000 per month. Long-Term Disability will continue to be payable for up to 2 years.

Accident

Accident Insurance is a way to help pay for medical bills and other out-of-pocket expenses that often arise after an unexpected injury. This policy is made available through Mutual of Omaha. This is a voluntary coverage and if elected will be paid by you, the employee, via a bi-weekly payroll deduction.

  • Attached is a full list of injuries and the amounts that will be paid out as well as information on how to submit a claim.

  • This benefit comes with a $50 Health Screening Benefit in which you are paid a lump sum of $50 when you complete a preventative health screening. This benefit can be paid once per year. If you complete this screening then it would pay for half that year of premium for the employee-only tier. Talk about a great benefit!

Hospital Indemnity

Hospital Indemnity is a way to help pay for medical bills and other out-of-pocket expenses that result from hospital-related fees. This policy is made available through Mutual of Omaha. This is a voluntary coverage and if elected will be paid by you, the employee, via a bi-weekly payroll deduction.

  • Attached is the policy details of the plan and information on how to submit a claim.

  • This benefit comes with a $50 Health Screening Benefit in which you are paid a lump sum of $50 when you complete a preventative health screening. This benefit can be paid once per year and can be completed by up to 6 family members per year.

401K

Your retirement plan is an important benefit you shouldn't overlook. It offers a powerful way to enhance your long-term financial well-being by investing in yourself. Match rates may vary from year to year at the company's discretion. For 2024, Tri-State will match 100% of the first 6% you contribute annually. 

  • Employees are eligible for this benefit the first of the month following 30-day of employment. You must be 20 years old to participate.

Employee Assistance Program

You don’t have to go through tough times alone, Tri-State’s EAP is here to help. Through Mutual of Omaha, all employees receive free EAP services. This benefit gives employees three counseling sessions per year (per household) via either face-to-face or video. 

  • The EAP can help with more than just standard counseling, it also includes resources for child and elder care, financial wellness, substance abuse and addiction, legal assistance and career help.
